Unique Dental Group, Inc. "Supporting social sector organizations within our communities" Grant Application Guidelines

Unique Dental Group, Inc. is a dental laboratory in Salt Lake City, Utah. The grant committee consists of employees and administrators. Please send your completed application to:

1. Grant proposals deadlines/grant awards announcements:

2. Project directors may be invited to present their proposal to the committee. All applications must be centered upon an innovative project or research. Areas of interest include scientific studies, human relations/education, and the medical industry. Funds will not be awarded for general organization expenses.

3. Unique Dental Group must receive a summary of the project's outcomes 30 days after the project is completed.

4. Unique Dental Group must be acknowledged on all formal communications concerning the project, and any published materials, booklets, or copyrighted material.

5. The Unique Dental Group Grant Committee welcomes proposals throughout the United States.

6. Unique Dental Group reserves the right to limit funding per quarter and/or not fund a quarter. If a quarterly grant is not funded, all proposals will be considered in the next quarter's funding.

7. Incomplete applications will not be considered.
